We are seeking a licensed Physical Therapist to provide in-home physical therapy services to patients throughout Falls Church, Alexandria, Arlington, and surrounding areas. This role offers autonomy with strong clinical and administrative support.

• Perform in-home physical therapy evaluations and reassessments

• Develop and implement individualized treatment plans

• Educate patients and caregivers on treatment plans and safety

• Document care accurately and timely in EMR

• Collaborate with an interdisciplinary care team

• DPT or MPT from an accredited Physical Therapy program

• Active Virginia Physical Therapy license

• CPR/BLS Certification

• Valid driver’s license and reliable transportation

• Strong communication and time-management skills

• Home health experience preferred, but not required
