- Perform comprehensive nursing assessments of patients in their homes, including physical, psychosocial, and environmental factors.
- Develop individualized care plans in collaboration with patients, families, physicians, and other members of the healthcare team.
- Provide skilled nursing care and interventions as prescribed (e.g., wound care, medication management, infusion therapy, catheter care, palliative care).
- Monitor patients’ health status, symptoms, and response to treatment, adjusting care plans as needed and communicating changes promptly.
- Educate patients and caregivers on disease management, medication adherence, self-care, and preventive health practices.
- Coordinate care with therapists, social workers, home health aides, and community resources to ensure holistic service delivery.
- Document assessments, interventions, and outcomes accurately in the EHR in line with regulatory and organizational standards.
- Provide emotional support and advocacy to patients and families coping with illness, disability, or end-of-life issues.
- Maintain strict infection control and safety practices in accordance with established protocols.
- Participate in interdisciplinary meetings, case conferences, and quality improvement initiatives.
- Graduate of an accredited nursing program with current licensure as a Registered Nurse (RN) in the state of employment.
- 2–3 years of nursing experience preferred (home health or community health strongly valued). Experience as an LPN will be considered.
- Strong clinical assessment, critical thinking, and technical nursing skills.
- Excellent communication and patient education skills with cultural sensitivity.
- Knowledge of home health regulations, standards of care, and best practices.
- Ability to work independently and manage a dynamic caseload effectively.
- Proficiency in electronic health record (EHR) systems.
- Experience with wound management and diagnosis preferred.
- Valid driver’s license, reliable transportation, and willingness to travel within the service area.
- Commitment to continuing education and patient-centered, compassionate care.
