• Collaborate with social services staff, interdisciplinary team, and administration to promote and protect patient rights, dignity, independence, and psychological well‑being.
• Maintain good working relationships with center employees and nursing staff to collaborate and plan.
• Assure a comprehensive psychosocial assessment is completed for each patient, identifying social, emotional, psychological needs and strengths.
• Educate patients and families regarding their rights and responsibilities, health‑care decision making/advance directives, effective problem solving, and available community resources.
• Provide therapeutic interventions to assist patients in coping with transition and adjustment to a nursing center and to address behavior or mood problems.

• Bachelor's degree in social work or human services required.
• Must possess any certifications/licenses as required by the state of employment to practice in long-term care.
