Clinical Pharmacy Specialist – Oncology/Hematology - Inpatient

Sanford Medical Center Fargo is seeking a Clinical Pharmacy Specialist in Oncology/Hematology for inpatient and outpatient oncology medication management. The pharmacy team is available 24 hours per day, seven days per week in the inpatient setting and works under established P&T protocols as well as collaborative practice agreements. The department collaborates with cardiology, dialysis, internal medicine, intensive care, emergency care, medication reconciliation, bedside medication delivery, OR pharmacy, transplant, and an antimicrobial stewardship program.

The hospital is a teaching center with opportunities to precept pharmacy residents and students.

40. Salary Range: $53.00 - $87.50. Pay Info:
Sign on Bonus: $15,000 with a commitment.

Responsibilities
  • Provide clinical pharmacy services for inpatient and outpatient oncology medication management in an interdisciplinary team environment.
  • Prospective medication order review for ambulatory outpatient oncology orders, with verification of high-risk medications for clinical, financial, and policy compliance.
  • Engage in decentralized inpatient oncology with team rounding; support pharmacokinetics, TPN monitoring and consultation, anticoagulation review, renal dosing, IV to PO adjustments, and medication profile review.
  • Participate in antimicrobial stewardship for the oncology patient population.
  • Contribute to committee work and projects to elevate oncology pharmacy practice.
  • Precept PGY-2 Oncology Pharmacy Residency, PGY-1 residents, PGY-2 residents, non-traditional residency program, and Advanced Practice Pharmacy Experiential students.
Qualifications
  • Graduate of an accredited School of Pharmacy; thorough knowledge of drugs and their actions; knowledge of state and local regulations.
  • Hospital experience or residency preferred.
  • License must be in good standing by the State Board of Pharmacy in the state(s) of practice; multi-state licensure privileges as required by position.
