Nurse Practitioner Opportunity – Primary Care / Home-Based Care

Locations: Fargo & Moorehead, ND

We offer a unique home-based care model, including primary care and palliative services, supported by full-time medical physicians and nurse practitioners who are on-call 24/7.



The Role

Seeking 2 experienced Nurse Practitioners to support growing patient panels in Fargo. These are full-time positions with a mix of home visits and assisted living facility visits. Providers have autonomy in scheduling their patient visits and maintaining a balanced work-life rhythm.

Responsibilities

• Provide high-quality primary and palliative care to geriatric patients in their homes and in assisted living facilities.
• Maintain autonomy over scheduling to best serve patient needs.
• Collaborate with medical team and participate in weekly team meetings.
• Patient load: 4–6 per day on average

Qualifications

• Current Nurse Practitioner license in North Dakota and Minnesota.
• Minimum of 3 years NP experience; primary care with geriatric population strongly preferred.
• Palliative or home-based care experience highly valued.
• DEA registration required.
• Ability to work independently in patient homes and manage a flexible patient load.
