Base Pay Range

$70,000.00/yr - $/yr

Job Requirements
  • Bachelor of Arts, Science Degree, Master’s Degree, or an Associate Degree in an allied health field, or comparable skilled nursing experience deemed adequate by Regional Director of Operations.
  • Current state licensure and/or certification/registry, as required.
Physical Requirements
  • Ability to sit for extended periods, walk through the department and office, and lift objects weighing up to ten pounds; must be able to climb stairs, stoop, and reach when performing daily work.
  • Must also meet physical requirements for the job description of held licensure (e.g., Physical Therapist).

Job Responsibilities
  • Responsible for the management and administration of all aspects of rehabilitation services, including maintaining a clinical caseload.
  • Responsible for recruiting and retaining all rehabilitation service personnel.
  • Ensures effective orientation and on-the-job training for all rehabilitation service personnel.
  • Promotes positive employee relations by facilitating an atmosphere of open communication and problem solving.
  • Responsible for conducting performance reviews of all rehabilitation service personnel.
  • Responsible for all aspects of operations of rehabilitation services at the facility level.
  • Monitors personnel compliance with all standards and takes corrective action when needed.
  • Works with finance personnel in reconciling billing to ensure accuracy.
  • Assures that all program standards are met, including MDH SNF standards and any other overseeing agencies.
  • Identifies program development needs of the community and implements appropriate programs.
  • Develops clinical affiliation programs with educational institutions.
  • Investigates, follows up, and assists in the resolution of staff, service, and resident-related issues as requested by the Administrator and/or the Executive Director.
  • Reviews the quality and appropriateness of the total services delivered.
  • Attends facility meetings as requested.
  • Assesses resources and supplies available in the community.
  • Tracks MDS Assessment Period Data, including number of disciplines treating, treatment days per week, and daily treatment time, and communicates data to appropriate community personnel.
  • Reviews resident‑specific PDPM category per assessment period and coordinates changes in MDS data according to community procedure.
  • Responsible for utilizing tracking systems and implementing changes that may be indicated.
  • Ensures compliance with daily resource utilization data entry and CPT coding, monitoring treatment minutes daily.
  • Assists with and participates in quality assurance programs as required.
  • Adheres to established confidentiality standards for residents and the organization.
  • Maintains appropriate relationships and rapport with coworkers, supervisors, residents, family members, and community personnel and administration.
  • Projects a positive and professional image at all times.
  • Generates special reports/data as needed to assist the Administrator and/or Executive Director and Team Leader in assessing the efficiency and cost‑effectiveness of rehabilitative services for the facility.
  • Reports to work on time and coordinates schedule to achieve maximum results of rehabilitative services operation within the facility.
  • Maintains a safe and clean work area and adheres to community safety standards, including Infection Control, Universal Precautions, and OSHA standards for the healthcare professional.
  • Performs other duties as assigned and as required to effectively perform the responsibilities of the position in the best interest of Northwoods Rehab.
  • Assumes responsibility for ongoing continuing education and professional development.
