Registered Nurse Case Manager – End-of-Life Care
Job Title: RN Case Manager – Dignity. Compassion. Clinical Excellence.
Pay: $90,000 – $130,000 DOE
Benefits: Full Benefits Package
Location: Greater Twin Cities Metro Area

What the Role Involves- Supporting patients through complex emotional and physical transitions
- Providing education and reassurance to families
- Coordinating a high-quality care experience from admission through bereavement
- Collaborating with social workers, aides, and medical directors
- Holds an RN license in MN
- Has prior hospice, oncology, or palliative experience
- Balances heart-centered care with medical skill
- Understands the sacredness of this work and approaches it with grace
