Overview

Home Health RN Admission Nurse role at Prairie River Home Care, Inc. This position reports to the Clinical Supervisor and involves providing client care in accordance with company Policies and Procedures, maintaining relationships with referral sources, and supporting admissions processes.

Schedule
  • This is a full-time position, Monday - Friday with an on-call rotation.
  • Travel to clients  homes is required to provide 1:1 client care that may take 1-3 hours per client.
Coverage
  • Reports to the Mankato, MN office and provides services in Faribault, Owatonna, Waseca, Albert Lea, and surrounding cities.

Major Responsibilities
  • Client Care:
    • Performs initial and periodic assessment of clients to determine needs. Implements and evaluates the effectiveness of care.
    • Uses interview and physical assessment skills to evaluate client needs.
    • Demonstrates nursing skills appropriate for services provided.
    • Records nursing diagnoses, measurable outcomes, interventions and nurses notes according to company standards.
    • Develops and updates care plans as client s condition indicates.
  • Demonstrates knowledge of current health practices when performing observation and evaluation of client care.
  • Provides total client care including medications and treatments ordered by the physician, physical and psychological care, and client and family education.
  • Assesses and interprets symptoms and institutes remedial measures within the legal limits of nursing practice in Minnesota; reports significant changes promptly.
  • Demonstrates problem solving in adapting to the client s changing needs.
  • Organizes assignments to provide quality care in a cost-effective manner.
  • Coordinates client records in the home and the office.
  • Coordinates client care with family, other caregivers, community resources and other disciplines.
  • Maintains confidentiality of all client, family, and employee information.
  • Maintains a safe client environment and reports suspected abuse, neglect, or financial exploitation.
  • Follows infection control policies.
  • Participates in after-hours on-call process to ensure client care policies and staffing needs are met.
  • Supervision and Leadership:
    • Teaches and evaluates delegated tasks to improve client care.
    • Provides orientation to employees per policy.
    • Keeps up to date on state and federal regulations for home care.
    • Participates in case conferences and team meetings in the branch office.
    • Maintains positive and effective communication with employees; supports a team concept.
    • Promotes a positive client and employee service image.
    • Demonstrates self-direction to prioritize and accomplish responsibilities.
    • Performs other related duties as assigned by the Branch Office.
  • Education and Teaching:
    • Assesses readiness and ability of the client and significant others to learn; delivers individualized teaching.
    • Documents teaching and client response.
    • Engages in ongoing professional growth through continuing education, in-service programs, orientation and evaluations.
  • Financial:
    • Ensures client authorizations for services are current and services meet payer requirements.
    • Applies basic knowledge of financial resources for care.
    • Completes admission and financial forms accurately and distributes them appropriately.
  • Community Relations:
  • Develops and maintains relationships with referral sources including physicians, hospitals, nursing homes, community resources and payers.
Physical/Environmental Demands

Medium to Very Heavy. Client care in homes; adaptable to various environments. Must be able to drive or use public transportation in all weather. Physical effort may include lifting and transferring clients.

Qualifications
  • Currently licensed as an RN in Minnesota; license must be active and without limitations or restrictions.
  • At least two (2) years of experience in a health care facility or Home Health Care field.
  • Strong organizational skills and ability to work independently.
  • Ability to make appropriate clinical judgments.
  • Excellent written and oral communication skills; interpersonal and teaching abilities.
  • Basic computer skills; knowledge of Word and Excel is a plus.
  • Understanding of Medicare certification and OASIS documentation experience required.
  • Ability to manage multiple assignments with priorities and ensure completion.
  • Access to dependable vehicle or public transportation for travel to multiple company sites daily.
  • Valid driver s license and proof of car insurance when using a personal vehicle for company business.
  • Proof of negative Mantoux or negative chest X-ray.
  • Current CPR certification recommended.
  • U.S. Citizenship or valid Alien Work Permit.
