Superior Hospice is seeking a Hospice RN Case Manager in the Greater Twin Cities area. You will manage your own caseload, perform comprehensive nursing assessments and develop individualized plans of care. Focus will be on pain and symptom management with compassionate, patient-centered care.
You will coordinate with an interdisciplinary team and provide care in patients' homes and care facilities, maintaining EMR documentation and Medicare compliance. Weekend/on-call rotation required.
