Location: Southmoor
Dorset Health Care's Mid Dorset Hub is seeking a Frailty Practitioner on a fixed-term 8-month secondment to enhance care for patients living with frailty. You will deliver proactive and reactive home-based care, work with the MDT, and help prevent hospital admissions.
The role runs 8:30–16:30 with potential weekend rota; DBS checks required; NHS service experience preferred;
Band 6 under Agenda for Change.
