The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with company policies, professional standards, and applicable regulations. This entry-level position coordinates patient services with nursing, monitors progress toward goals, and educates patients, caregivers, staff, and other professionals regarding appropriate medication use.

• Determine the suitability of individual patients for home care, collect and organize all necessary patient information, and assess suitability per company policies.
• Design, recommend, monitor, and evaluate patient‑specific pharmacy care plans, including adjustments to medication therapy and monitoring parameters.
• Modify pharmacy care plans based on monitoring data and patient information; use pharmacokinetic principles and interpret laboratory values.
• Provide medication‑use education to staff, professionals, patients, and caregivers using effective techniques.
• Ensure continuity of pharmaceutical care between home and other patient‑care settings, including communication of pertinent information.
• Prepare and dispense medications following policies and procedures, ensuring proper concentration, compatibility, stability, labeling, and safety.
• Manage medication administration equipment, troubleshoot issues, participate in incident reporting, and complete quality assurance documentation.
• Respond to drug‑information requests from providers and patients in a timely manner, citing literature and maintaining CEU compliance.
• Manage investigational drug products per regulations and policies.
• Comply with accreditation, legal, regulatory, and safety requirements, and maintain patient confidentiality.

• Graduate of an accredited School of Pharmacy.
• Current, active pharmacy license issued by the state board.
• One of the following: a year of experience in sterile IV admixture procedures, knowledge of non‑sterile compounding, immunizations, and MTM, completion of an ASHP accredited residency program, completion of an advanced pharmacy practice experience, or equivalent experience approved by the VP of Pharmacy.

• Experience in providing customer service to internal and external customers with a focus on quality standards and customer satisfaction.
• Basic proficiency in Microsoft Word, Excel, and PowerPoint.

Willing to travel up to 5 % of the time for business purposes within the state.

• Completion of an ASHP accredited residency or advanced pharmacy practice experience.
• Experience in home care, hospital, or other clean‑room settings.
