• Under provider direction, provide skilled nursing care in a variety of clinical settings, using the nursing process to assess, plan, intervene, and evaluate human responses to health problems.
• Administer office‑based nursing care in collaboration with providers.
• Assess physical and psychosocial needs and environmental factors affecting patients and families, formulate nursing plans, and ensure continuity through appropriate evaluation and intervention.
• Perform independent nursing actions based on assessment and problem identification.
• Triage telephone calls, initiating appropriate nursing intervention in a prompt, precise, and professional manner.
• Assist patients and family members with concern and empathy; respect confidentiality and privacy.

Travel to other regional offices as needed.

• Current state of Connecticut RN license and current CPR certification.
• Three years of experience in a medical practice preferred.
• Experience with medical software.
