• Client Specifics 3 year old with a trach vent and a g tube

• Hours 12 hour Overnight shifts
  
  Overnights Every Sunday Wednesday & Saturday night
  
  Overnights Every other Thursday & Friday night
• Client Specifics 1 year old with a trach vent and a g tube

• Provide skilled nursing care in home and school settings
• Assess and monitor your patients condition and communicate changes with the care team
• Administer medications treatments and prescribed therapies
• Provide trach ventilator airway and G tube care as needed
• Maintain accurate documentation and support families through education
• Follow individualized care plans and maintain a safe care environment
