Kinship Caregiver Navigator (ALTSD #)

New Mexico Aging & Long-Term Services Department

Pay

Base pay range: $45,680.00/yr – $68,520.00/yr ($21.96 – $32.94 hourly). Position is Pay Band C5.

Mission

The Long‑Term Care Division (LTCD) oversees community‑based programs that help older adults thrive in their homes, reduce nursing home placements and emergency department visits.

Responsibilities
  • Oversee program rules and eligibility.
  • Provide technical assistance and support to participants, caregivers, and contractors.
  • Resolve complex client issues.
  • Conduct in‑home visits and complete assessments.
  • Track and report performance measures accurately and timely.
  • Recruit participants and determine eligibility.
  • Conduct program presentations.
  • Track and monitor participant cases, including care plans and follow‑up.
  • Connect families with local resources (financial aid, legal support, health care, etc.).
  • Make referrals and provide ongoing support through virtual and in‑person engagement.
  • Maintain assigned caseloads, outreach to participants, and coordinate with partners.
  • Advocate for services and assist families in accessing benefits.
  • Maintain accurate and timely case records.
Qualifications
  • High school diploma or equivalent plus two (2) years of experience as a social work assistant or in social or community coordination, working with health or social service matters, or behavioral health care; or a combination of education and experience totaling two (2) years.
  • Experience working with high volume caseloads, guiding and directing clients.
  • Experience in home‑ and community‑based programs.
  • Knowledge of local and statewide programs supporting older adults and children.
  • Proficiency with Word, Excel, PowerPoint, and case‑management systems.
  • Experience implementing policies and procedures.
  • Overnight stay required.
Employment Requirements
  • Valid driver’s license.
  • Pre‑employment background investigation.
  • State and federal criminal background check.
  • Current Defensive Driving Course Certificate from New Mexico or certification within 90 days of employment.
Working Conditions

Work is performed in an office setting and in the community, using personal computers, telephone, fax, copier, and other office equipment. Occasional after‑hours and weekend work may be required. Statewide travel is expected.
