Salary: $24.16 - $36.24 hourly ($50,248 - $75,372 annually). This position is a Pay Band C6.
Location: Farmington Public Health Office, 355 S. Miller, Farmington, NM 87401.
Working as a Children's Medical Services Social Worker involves:
- Assisting families of children (ages 0‑21) in developing coordinated plans to meet medical, social, and developmental needs.
- Conducting outreach and engagement of clients/families.
- Providing comprehensive biopsychosocial assessments and periodic reassessments.
- Developing and implementing service plans.
- Coordinating and monitoring service delivery and advocating on behalf of the client.
- Collaborating with medical, educational, vocational, and human service providers.
- Providing outreach to immigrant and Native American/Pueblo communities.
- Ensuring quality assurance for billing and maximizing billing on care coordination.
Ideal Candidate: All candidates must possess an active LBSW, LMSW, or LCSW. Preferred experience includes conducting intake interviews, providing crisis interventions, formulating interventions and service plans, working with families in medical/public health settings, and fluency in English and Spanish.
Minimum Qualification: Bachelor's degree in Social Work (in accordance with NMAC 61‑31‑9) and two (2) years of experience in a social work setting, or an equivalence combination totaling six (6) years.
Employment Requirements:
- Valid Driver's License.
- Current Defensive Driving Course Certification from the State of New Mexico, or completion of a defensive driving course upon hiring.
- Active LBSW, LMSW, or LCSW license.
Working Conditions:
- Services provided in communities, clinics, auxiliary sites, and consumer homes to a diverse population, many of whom do not speak English.
- May require lifting up to 50 pounds.
- Potential exposure to communicable diseases.
- Travel and occasional overnight stays required.
- May involve work outside traditional hours to provide outreach.
