Join to apply for the LPN II role at Centra Health. The Clinic LPN provides patient care under the direction or supervision of a licensed medical practitioner, a professional registered nurse, or other licensed health professional, in outpatient or office settings. The LPN collaborates with providers and the interdisciplinary team, maintains a safe patient environment, and coordinates care processes to achieve desired outcomes.

The practice aligns with the American Nurse Association’s Code of Ethics for Nurses, the Nurse Practice Act of Virginia, and the ANA Principles of Nurse Staffing.

• Clinical – Assessment: Conduct individualized patient assessments as determined by patient condition, policies, procedures, protocols, and/or RN provider direction; reprioritize and implement appropriate patient care accordingly.
• Clinical – History & Interventions: Interview patients to obtain health histories and medication histories; integrate nursing interventions into the multidisciplinary plan of care; apply appropriate standards of care; practice patient outcomes.
• Clinical – Intervention: Perform treatments as ordered by provider; implement individualized plan of care for the patient and family; recognize urgent or emergent risk and initiate appropriate responses; provide culturally sensitive care; collaborate with the interdisciplinary team; administer medications and vaccines per Centra policy; assist in medication refills; participate in specimen collection and processing.
• Clinical – Evaluation: Evaluate patient responses to care; adjust the care plan as needed; document assessments and progress; provide health promotion and patient education.
• Self and Peer Learning: Maintain current competencies; participate in required education and professional development; stay engaged with nursing literature and professional networks.
• Resource Utilization & Safety: Support appropriate resource use; serve as preceptor/mentor; assist in orientation and onboarding of staff and students.
• Quality & Ethics: Participate in quality assurance activities; identify practice concerns; uphold Centra safety behaviors; maintain patient confidentiality and advocate for patients.

• Completion of accredited LPN training program (required)
• At least one year of nursing (LPN) experience
• Prefer prior experience in a similar setting
• Valid, current LPN license in the State of Virginia (Required)
• AHA BLS Course Completion
• ACLS may be required for LPNs at satellite clinics within the Heart and Vascular Institute
• LPNs working for the CMG Resource team may be required to travel
