Registered Nurse – Living Well
Location:
Farnham, Surrey with cross site working.
Salary: £33,580-£37,810 per annum WTE (based on 37.5 hours per week). Salary dependent on experience.

A registered nurse who is committed to supporting patients, families and carers to live well with advanced and terminal illness is required. The Living Well Service provides holistic, physical, emotional, psychological, and spiritual care and support to people living with advanced or terminal illness who are stable or slowly deteriorating. Care is delivered in settings relevant to patients’ needs, including sites, groups, outpatient, virtual, community or home visits.
The successful applicant will work as part of the North East Hampshire, Guildford, Waverley and Surrey Heath multi‑disciplinary locality teams and in partnership with community services, specialists and primary care to ensure care is coordinated, holistic and person‑centred.
The postholder will support patients to manage symptoms, take a proactive approach to enabling choices, support self‑management and will be supported to develop their skills in palliative care. The role requires flexibility to work between sites such as The Beacon and The Hospice in Farnham or other service areas according to patient need.
Responsibilities- Provide high quality holistic, person and family centred care.
- Support patients and families to manage symptoms and make informed choices.
- Coordinate and collaborate with multi-disciplinary teams and community services.
- Participate in continuous learning and professional development in palliative care.
- Current NMC registration in adult or general nursing.
- Commitment to enabling choice and supporting a person’s autonomy.
- Strong communication, interpersonal and team working skills.
- Willingness to undertake further training in communication and palliative care skills.
- Understanding of community working.
- Self‑awareness, resilience and ability to cope with stressful situations.
- Post‑registration experience, preferably with people approaching end of life.
