Physical Therapist; PT - Home Health Visits

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a per diem home health position in our Fayetteville, NC office.

This role services adult and geriatric clients in their homes across Lillington & Fayetteville areas.

As a home care Physical Therapist, you will be part of a multidisciplinary team providing rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities include:

• Making home visits within designated territories.
• Performing diagnostic tests and measurements related to mobility, strength, balance, and safety.
• Developing and implementing individualized care plans with manual therapy, gait training, and other interventions.
• Assessing and revising care plans, collaborating with nursing staff.
• Educating clients and families on rehabilitative activities.
• Documenting care activities accurately using electronic devices.
• Implementing wound care interventions using evidence-based dressings and topical agents.

Qualifications:

• Current North Carolina PT license.
• Graduation from an accredited Physical Therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Proficiency with computers; EMR experience preferred.
