The Care Manager (RN) develops, assesses, and facilitates complex care management activities for members with physical needs, aiming to provide high-quality, cost-effective healthcare outcomes through personalized care plans and education.

• Evaluate the needs of the member and recommend a plan for the best outcome.
• Develop ongoing care plans and collaborate with providers to address unmet needs.
• Identify problems/barriers to care and provide appropriate interventions.
• Coordinate between the member and care provider team for person-centered care.
• Provide ongoing follow-up and monitoring of member status and care plan goals.
• Facilitate care management and collaborate with providers for timely access to care.
• Perform telephonic, digital, home, and/or other site outreach to assess member needs.
• Collect, document, and maintain member information to ensure compliance.
• Provide education to members and their families on disease processes and care options.
• Provide feedback to leadership on opportunities to improve care delivery.

Education:

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing required.

Experience:

• 2 – 4 years of related experience required.

Licenses &

Certifications:

• RN
  - Registered Nurse
  - State Licensure and/or Compact State Licensure required.
