Summary
Responsible for the development, implementation and documentation of the antimicrobial stewardship program for Cape Fear Valley Health System. Ensure clinical and microbiological effectiveness of antimicrobials; decrease the emergence of resistance; educate prescribers (including pharmacists, nurses, and ancillary healthcare providers) in therapeutically efficacious therapy; decrease adverse reactions and predisposition to secondary infections; promote conversion of intravenous therapy to oral antibiotics; consolidate antimicrobial formulary.

Responsibilities- Performs comprehensive patient medication order evaluation, check for problems with dosages, interactions, and incompatibilities
- Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols
- Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes as necessary
- Assist with pharmacokinetics consult services and drug dosing per organizational protocols
- Detects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner set by medication safety pharmacist; documents all clinical activities and interventions accurately and completely; works independently with minimal supervision; organizes and prioritizes work assignments, and ensures pharmacy services are provided in a timely manner based on team responsibilities that are reflected by peer benchmarking and set by clinical services manager
- Answers the telephone, identifying self and department; directs calls to appropriate personnel, and in person drug information requests when necessary; attends and successfully completes orientation, education, and training programs; reviews literature and other materials pertinent to the practice of pharmacy
- Completes all competence/skills assessment requirements
- Performance demonstrates efforts to improve patient satisfaction, lower costs, improve quality, and promote safety
- Is punctual and dependable; reports to work as scheduled; fulfills on-call obligations per prearranged schedule; absenteeism and tardiness are within policy guidelines
- Develops working relationships with pharmacists, attending physicians, medical residents, physician assistants, nurse practitioners, nurses, and other health care team members in the Cape Fear Valley system
- Creates a sustainable rounding schedule to provide clinical pharmacy services to patients as part of the medical team
- Perform work within specific time frames; adapts positively to frequent interruptions and changes in workload and/or work schedule; demonstrates sound professional judgment consistent with clinical/academic background
- Functions as a supervisor of residents, students, and co-workers
- Maintains professional development requirements set by ASHP to be a qualified preceptor
- Develops and implements quality improvement measures to improve patient care and staff efficiency
- Precepts, evaluates, and serves as a mentor to pharmacy learners including PGY1/PGY2 pharmacy residents and Pharm
D students
- Doctor of Pharmacy Degree required
- Current, valid licensure with the North Carolina State Board of Pharmacy required
- 3 years clinical practice experience after obtaining a Doctor of Pharmacy degree or completion of ASHP residency required
- Must be able to speak, read and follow instructions in English
- Must have excellent verbal and written communication skills
- Must have excellent presentation skills
- Good computer skills and working knowledge of Microsoft Excel, Word and Power Point
- Ability to see and read physician orders, medication labels, etc.
- Manual dexterity to use computer and typewriter
- Ability to communicate effectively with co-workers and patients
