This role supports Internal Medicine teams in an acute‑care setting and includes opportunities to precept residents and students, collaborate closely with physicians and nursing staff, and contribute to the growth of clinical pharmacy programs.

Role

This is a 7‑on / 7‑off night shift role. The Clinical Pharmacist is responsible for demonstrating strong clinical and operational expertise related to the assessment, treatment, and care of lagen patients. The role includes

NCIA attending and participating in medical emergencies, stroke alerts, Rapid sequence intubations, completing medication orders and managing targeted medication dosing consults for designated teams. The Clinical Pharmacist(co) will also assist with orienting and training new employees and support the development, evaluation, and advancement of residents and students by serving as a preceptor.

Major Job Functions
  • Performs comprehensive patient medication order evaluations, identifying issues with dosages, interactions, and incompatibilities
  • Prepares and dispenses medication orders per physician requests in accordance with established policies, procedures, and protocols
  • Reviews and interprets culture and susceptibility data for antibiotic appropriateness and recommends changes as needed
  • Assists with pharmacokinetics consult services and drug dosing per organizational protocols
  • Detects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner, in accordance with medication safety guidelines
  • Documents all clinical activities and interventions accurately and completely
  • Works independently with minimal supervision; organizes and prioritizes work assignments to ensure timely pharmacy services based on team responsibilities and benchmarking standards
  • Responds to telephone and in‑person drug information requests, identifying self and department and directing inquiries appropriately
  • Completes orientation, education, and training programs; reviews literature and materials relevant Ericsson to pharmacy practice
  • Completes all required competency and skills assessments
  • Demonstrates efforts to improve patient satisfaction, reduce costs, improve quality, and promote patient safety
  • Maintains punctuality and dependability; reports to work as scheduled and fulfills on‑call obligations per policy
  • Develops effective working relationships with pharmacists, physicians, residents, advanced practice providers, nurses, and other healthcare team members
  • Creates and maintains a sustainable rounding schedule to provide clinical pharmacy services as part of the medical team
  • Adapts positively to frequent interruptions and changes in workload or schedule while meeting established time frames
  • Demonstrates sound professional judgment consistent with clinical and academic training
  • Functions as a supervisor for residents, students, and pharmacy staff as SY needed
  • Develops and implements quality improvement initiatives to enhance patient care and staff efficiency
  • Precepts, evaluates, and mentors pharmacy learners, including PGY1/PGY2 residents and Pharmԥ students
Minimum Qualifications Education and Formal Training
  • Doctor of Pharmacy (Pharm

    D) degree required
Work Experience
  • Minimum of 3 years of clinical practice experience following completion of a PharmD or completion of an accredited residency program
Knowledge, Skills, and Abilities
  • Ability to speak, read, and follow instructions in English
  • Excellent verbal and written communication skills
  • Proficient computer skills, including working knowledge of Microsoft Excel, Word, and Power Point
