Position: Geriatric Nurse Practitioner opening in Fayetteville, NC - Generous benefits package

Seeking a Nurse Practitioner to provide effective and equitable value-based primary care to Medicare patients in Fayetteville, North Carolina.

Details:

  • Full-time, Mon - Fri, 8am-5pm
  • 13-16 patients per day
  • Geriatric population
  • Provision of exceptional primary care.
  • Conduct office visits for routine and acute issues.
  • Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation.
  • Care coordination with other providers, specialists, testing facilities, and agencies.
  • Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years)
  • Assisting the care team with phone triage and outreach.
  • Conducting home visits as needed

Qualifications:

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate required
  • National certification as a Family, Adult or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC
  • Experience in primary care:
    Internal medicine, geriatrics, or family medicine preferred
  • Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).
