RN Field Case Manager

Role Overview

RN Field Case Manager Health Care  Support is actively seeking a RN Field Case Manager to fill an opening with a prestigious Healthcare Organization in Fayetteville, NC.

Principal Duties and Responsibilities:

• Ensure accurate and complete documentation.
• Communicate and collaborate with the interdisciplinary care team.
• Assist the provider/team with various care coordination activities.
• Assist the provider/team with benefit determination associated with Medicare part A, part B, and part D benefits.
• Responsible for collaborating with the primary advanced practice clinician (APC) and facility to identify and respond to patient change in condition.
• Assist the provider / team with therapy coordination.
• M-F: 8a-5p.

• Clear and active NC RN license.
• 1-2 years of experience.

• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs.
• Understanding of Geriatrics and Chronic Illness.
• Proficient computer skills including the ability to document medical information with written and electronic medical records.
• Effective Communication skills and time management.

Pay Details: $45.00 – $50.00 an hour
