Enrich the lives of older adults through community, choice, and continuing care in the Licensed Clinical Social Worker role.The Clinical Social Worker identifies, assesses, and manages the psychosocial needs of patients and families relating to home health services, terminal illness and end‑of‑life care, and develops and implements appropriate care plans as part of an interdisciplinary team. Social work services are based upon the patient’s psychosocial assessment and the patient’s and family’s needs and acceptance of these services. Provides patients and families with counseling, bereavement counseling, education, serves as family and patient advocate and arranges consultations when needed related to hospice, palliative care and end‑of‑life.

- Provide medical social services to Home Health and Hospice patients and their families following established protocols.
- Identify, assess, and support patients’ emotional, social, spiritual, and cultural needs.
- Assist patients and families in accessing home‑based and community resources.
- Act as a liaison with external agencies such as SSA, VA, and DSHS.
- Assess family financial situations and facilitate referrals for financial assistance when appropriate.
- Conduct initial and ongoing psychosocial assessments for Home Health, Hospice, and bereavement services.
