Collaborative Care Mental Health Therapist - Primary Care

Multi Care Health System is seeking an Integrated Mental Health Therapist (IMHT) to join a multidisciplinary team embedded in the primary care clinic. The IMHT provides evidence-based behavioral health services in partnership with the treatment team to promote symptom reduction and optimal patient outcomes.

You will assess symptoms, develop treatment plans, monitor progress, support medication management with PCPs, track outcomes, and coordinate care through warm handoffs during medical visits.
