We are seeking a dedicated Hospice Registered Nurse to provide compassionate hospice care to patients throughout King County. Care is delivered across a variety of settings, including private homes, adult family homes, assisted living communities, and skilled nursing facilities. In this role, you will coordinate and manage end-of-life care by addressing immediate and ongoing patient needs, managing symptoms, and ensuring each patient receives high-quality, dignified care that aligns with their goals, values, and wishes.

- Perform initial and ongoing comprehensive assessments of patients' physical, emotional, and spiritual needs and develop individualized plans of care.
- Collaborate with physicians, social workers, volunteers, and interdisciplinary team members to implement, coordinate, and evaluate care plans.
- Administer medications and therapeutic interventions to effectively manage pain and symptoms, promoting comfort and quality of life.
- Educate patients and families on the hospice philosophy, disease progression, symptom management, self-care, and end-of-life expectations.
- Provide emotional support and counseling to patients and families experiencing grief, anxiety, or other psychosocial concerns.
- Maintain accurate, timely, and compliant clinical documentation reflecting care provided and changes in patient condition.
- Ensure care delivery complies with Medicare guidelines, state regulations, and hospice policies and standards.
- Advocate for patient dignity, preferences, autonomy, and rights in all aspects of care.
- Respond promptly to urgent patient needs and collaborate with the on-call team as necessary.
- Participate in quality assurance, performance improvement, and interdisciplinary team meetings.
- Participate in scheduled weekend rotations
- Current, active Washington State RN license in good standing
- Active BLS certification
- Valid driver's license and reliable transportation
- Minimum 1 year nursing experience in hospice or home health preferred
- Minimum 2-3 years of healthcare related experience required
- Proficient with EMR systems and healthcare technologies
- Ability to exercise sound judgment, initiative, and work independently within scope of practice
- Adaptable to diverse social, cultural, and religious backgrounds
