Pathways to Housing DC is seeking an Outreach Case Manager to join the GTBID Homeless Outreach Team in Washington, DC. You will conduct assessments, coordinate housing and supportive services, and build relationships with individuals experiencing homelessness to promote housing stability.
The role combines field-based work (about 75%) with administrative duties, including documentation and care coordination, and requires strong communication, organization, and crisis intervention skills.
