** Position Summary - Physician
** The Family Physician assesses and evaluates the health care needs of patients, performs diagnostic and therapeutic procedures, manages care, and contributes to quality improvement initiatives. This position is ideal for a mission-driven physician seeking meaningful, relationship-based care in a rural community setting.
** Essential Duties and Responsibilities
*** Conduct patient histories and comprehensive physical examinations.
* Diagnose, treat, and manage acute and chronic medical conditions.
* Order and interpret laboratory and diagnostic tests.
* Formulate individualized treatment plans based on clinical findings.
* Educate patients and families on preventive health, lifestyle, and chronic disease management.
* Collaborate with behavioral health, dental, and pharmacy teams to ensure integrated care.
* Participate in peer review, quality improvement initiatives, and provider meetings.
* Supervise and mentor Advanced Practice Clinicians and students when requested.
* Maintain accurate and timely documentation in the electronic health record (EHR).
* Participate in after-hours call rotation and community engagement efforts.
** Qualifications

** Education and Experience
*** Completion of an accredited Medical School and Family Medicine Residency.
** Certificates, Licenses, and Registrations
*** Current California Medical License (MD or DO).
* Board Certification in Family Medicine.
* Current DEA License and CPR Certification.
** Position Summary - APC
** The Nurse Practitioner / Physician Assistant assesses and evaluates patient health needs, performs diagnostic and therapeutic procedures, manages care, and contributes to the overall quality and effectiveness of clinical services. This position is ideal for a clinician who values teamwork, patient relationships, and the opportunity to make a lasting impact in a rural community.
** Essential Duties and Responsibilities
*** Conduct patient histories and comprehensive physical examinations
* Diagnose, treat, and manage acute and chronic medical conditions
* Order and interpret laboratory and diagnostic tests per approved protocols
* Consult and collaborate with physician supervisors as needed
* Develop and coordinate individualized care plans
* Provide patient education on disease prevention and wellness
* Participate in quality improvement, required trainings, and peer review
* Mentor peers and precept students when requested
* Maintain accurate, complete, and timely documentation in the EHR
* Participate in after-hours call rotation and site committees as needed
** Qualifications
* ** Commitment to equitable, patient-centered care
* Strong communication, collaboration, and critical thinking skills
* Ability to serve patients with compassion and cultural sensitivity
** Education and Experience
*** Graduate of an accredited Nurse Practitioner or Physician Assistant program
* Credentialed through the American Nurses Association or Physician Assistant Association
** Certificates, Licenses, and Registrations
*** Current California medical license (NP or PA)
* Current Board Certification
* Current DEA License and CPR Certification
