Registered Nurse, Home Health

As a Home Health Registered Nurse
, you will:

• Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
• Confer with physicians to develop and revise the initial plan of treatment based on orders and assessments, under supervision.
• Coordinate care with healthcare personnel and external providers.
• Report patient progress to physicians and Clinical Managers regularly.
• Implement care plans collaboratively with patients and families to address their needs.
• Supervise and document the work of Home Health Aides and related staff, ensuring quality and compliance.
• Discharge patients with proper documentation after consultation.
• Maintain comprehensive medical documentation, including updates, care plans, and communication with other disciplines.

Required Experience/

Skills:

• Diploma, Associate, or Bachelor Degree in Nursing.
• At least one year of nursing experience preferred, with strong med-surg, ICU, ER, or acute care background.
• Home health experience is a plus.
• Valid RN license and CPR certification.
• Strong organizational and communication skills.
• Valid driver’s license, auto insurance, and reliable transportation.

Pay Range: $55.00 - $76.00 per visit/unit; $80,400 - $110,500 annually, depending on location and experience.
