Behavioral Health Coordinator
Position Summary
This individual will provide behavioral health support to residents of long‑term care facilities under the supervision of a licensed professional. The role includes training in assessments and interventions that support treatment specified in individual care plans. Regional travel will be required.
- Conduct assessments that include the behavioral and social needs of patients.
- Meet with families and facility staff to coordinate care as recommended by licensed professionals.
- Assess patients’ knowledge of their clinical condition and provide education and self‑management support.
- Provide outreach to clients who are having problems with treatment adherence.
- Assist in the development of the individual goals and behavioral health treatment plan.
- Coordinate with other agencies and community providers.
- Maintain timely documentation of work.
- Serve as an active member of the behavioral health treatment team.
- Bachelor’s degree in Counseling, Social Work, Psychology or a related field.
- Experience in a psychiatric or long‑term care setting preferred.
- Customer service experience preferred.
- Ability and willingness to function independently and proactively in a long‑term care setting.
- Effective use of electronic medical records and Microsoft software (Teams, Excel, etc.).
- Skilled in written and verbal communication.
- Skilled in exercising a high degree of initiative, judgment and discretion.
- Ability to handle confidential and sensitive information.
- Must be able to sit or remain in a stationary position for extended periods while communicating, utilizing the computer, and interacting with others.
- May need to stand, bend, stoop, kneel or crouch to access files or equipment.
- Requires continuous operation of computers, phones and other office equipment.
- Noise level is usually low to moderate.
- Requires desk work in an office environment.
- Must be able to interact effectively with others, manage conflict, and handle stressful situations and deadlines.
- Ability to work flexible hours as needed to meet deadlines.
- Must possess a respectful and professional demeanor at all times.
