Hours:
Weekdays, 8a-5p
- 0.6 FTE position, weekdays
- Autonomous and collaborative provider serving as a "float" across Community Health Network hospitals (Community Hospitals North, East, South, Community Heart & Vascular, Community Howard Regional, and Community Hospital Anderson) to deliver palliative and health care services.
- Focus on comfort measures, end‑of‑life care, symptom management, and assistance with consults and new admissions.
- Flexibility to travel between hospitals in Indianapolis.
- Competitive compensation and benefits package, including incentive potential.
- 1‑3 years’ experience as a licensed Nurse Practitioner in palliative care.
- Comfortable assessing patients independently.
- Compassionate caregiver with excellent organizational, communication, and critical‑thinking skills.
- Strong interpersonal and teamwork skills, plus proven consultant/educator experience for patients and families.
Educational background consistent with requirements for licensure and certification within the state of Indiana.
