Registered Nurse (RN) Case Manager, Home Health (Part time) administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction of the Clinical Manager.

Competitive pay: $43 - $45 per hour. $2000 sign-on bonus; $1000 at first pay period and the rest on successfully completing six months at the agency in good standing.
What you’ll be doing:
Make the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
Initiate the plan of care and necessary revisions.
Provide those services requiring substantial specialized nursing skills.
Initiate appropriate preventative and rehabilitative nursing procedures.
Support the coordination of interdisciplinary care.
Prepare clinical and progress notes for each patient visit and summaries of care conferences on their patients in a timely manner.

Graduate of an accredited school of professional nursing.
Currently licensed as a registered nurse in Indiana.
Minimum of one (1) year of nursing experience in a home health setting (desirable).
Must have current BLS Certification.
Access to reliable transportation.
