Home Health LPN (9648) - Summit Home Care & Hospice
Full-time Licensed Practical Nurse (LPN) role in the Home Health team.
Shift: Dayshift, Monday – Friday (40 Hours/Week)
Pay Rate: $28.00 - $31.00 per Hour
Location: Fishers, IN & Surrounding Areas
Expected Travel: ~30 Mile Radius (Mileage Reimbursement, $0.48/Mile)
Sign-On Bonus: $3,000 (Paid in 2 installments)
- Provide direct patient care under the supervision of a registered nurse.
- Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes.
- Implement the plan of care initiated by the registered nurse.
- Provide accurate and timely documentation consistent with the plan of care.
- Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
- Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinic Manager.
- Use and/or prepare equipment and supplies effectively and efficiently adhering to aseptic techniques.
- Participate in personal and professional growth and development.
- Perform other duties assigned by the registered nurse.
- Graduate from an accredited practical nurse or vocational nursing program.
- Must hold a current license as a Practical Nurse (LPN) or Vocational Nurse (LVN) in Indiana.
- Current licensure in state and CPR certification.
- Must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
- Recent TB test, required.
- 3 years of nursing experience, preferred.
- Prior experience in community/home health and/or medical/surgical, preferred.
- Comply with accepted professional standards and practices.
- Demonstrates good verbal and written communication, and organizational skills.
- Must be able to lift, position or transfer patients and lift supplies and equipment.
