Days Per Month: 3-day coverage for this assignment
On-Call/Weekends: 24-hour call

Submission Requirements:
Board Certified or Board Eligible in ENT
Active Arizona medical license
DEA license
Minimum 1 year of experience
BLS/ACLS as required by hospital policy
Credentialing available on an emergency basis
