Base pay range

Position SummaryThe Quality Assurance Nurse (RN or LPN) is responsible for overseeing quality assurance, staff education, and infection prevention activities at the Arizona State Veterans’ Home – Flagstaff. This role ensures compliance with state, federal, and VA regulations while supporting continuous quality improvement initiatives.
Key Responsibilities Quality Assurance & Compliance- Implement and monitor Quality Assurance Performance Improvement (QAPI) programs
- Conduct audits, interviews, observations, and data reviews to ensure regulatory compliance
- Review monthly CASPER reports and develop Performance Improvement Plans (PIPs)
- Prepare and present QA reports, statistical analyses, and compliance summaries
- Assist leadership with corrective action plans and deficiency resolution
- Coordinate and prepare the facility for regulatory surveys
- Investigate incidents of non-compliance, abuse, neglect, or mistreatment
- Serve as Infection Preventionist for the facility
- Track, trend, and map infections using McGeer and non-McGeer criteria
- Conduct daily infection control surveillance rounds
- Ensure compliance with infection-related care plans
- Stay current with CMS and CDC guidance and best practices
- Identify staff training needs related to quality assurance and compliance
- Develop, implement, and evaluate staff education programs
- Maintain training and competency documentation
- Chair and coordinate Quality Assurance and Infection Control Committee meetings
Knowledge Of:
- Nursing home regulations (OBRA, state, federal, VA)
- Quality assurance and utilization review principles
- Facility policies, procedures, and operations
- Basic statistical and data analysis methods
- Adult learning and staff training methodologies
Skills In:
- Verbal and written communication
- Analytical thinking and problem-solving
- Data interpretation and reporting
Ability To:
- Work collaboratively across all levels of the organization
- Develop and coordinate training programs
- Active Arizona RN or LPN License
- DPS Level 1 Fingerprint Clearance Card
- CPR/BLS Certification
- 2-Step TB Test (clear results required)
- Must be local to Flagstaff, AZ at time of submission
- Must be available for in-person interview within 1 week of posting close
- Must be able to start within 2 weeks of offer
- Resume (must list current location)
- CPR/BLS Card
- TB Skin Test / 2-Step TB Test
- RN or LPN License
- Hepatitis B Vaccination or Declination
