Wound Care Nurse (RN or LPN)

Base: $63.86–$65.00/hr actual pay will be based on skills and experience.

We are hiring a single Wound Care Nurse to work in a long‑term care environment. The role focuses on wound assessment, prevention, and ongoing care with interdisciplinary collaboration.

Schedule: Flexible. 7am‑3pm, 8am‑4pm, or 8am‑5pm. 8‑hour shifts Monday‑Friday. No weekends.

• Perform wound assessments and management for all patients per policies and procedures.
• Develop skin‑care programs to prevent skin breakdown of residents.
• Participate in developing and implementing policies for managing residents with draining wounds, fistulae, and tubes.
• Revise plan of care based on resident response and evaluate overall effectiveness.
• Assess and manage pain, utilizing appropriate techniques and educating residents and family.
• Serve as a resource by providing in‑services on wound prevention, wound care, ostomy care, and incontinence.
• Communicate clearly to nursing staff and physicians.
• Consult other healthcare professionals for an interdisciplinary approach.
• Ensure documentation meets current standards and practices.
• Attend training and continuing education in wound prevention, management, and ostomies, staying current with advances.

• Current license to practice as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in Arizona.
• Certification as a Wound, Ostomy, and Continence Nurse (CWOCN or CWCN).
• Must obtain and retain a fingerprint clearance card issued by the Arizona Department of Public Safety.
• Current CPR card obtained by physical assessment.

• Finger‑print clearance card:
  Level 1.
• CPR/BLS card.
• TB skin test.
• Hepatitis B vaccination or declination of offered vaccine.
• Competency test.
• Drug test.
• COVID vaccine card.

• Experience in a skilled nursing facility or long‑term care environment.
