The Department of Economic Security, Division of Developmental Disabilities seeks a Case Manager 3 to support individuals with developmental disabilities and their families in Flagstaff. The role involves conducting home and site visits, developing person-centered service plans, facilitating meetings, collaborating with community agencies, and maintaining case records. The position requires a valid driver's license and demands knowledge of case management practices, HIPAA compliance, and community resources.
Keyqualifications:
- Arizona licensed Registered Nurse OR Bachelor's degree in Social Work, Psychology, Special Education, or Counseling with 1+ year case management experience
- Minimum 2 years case management experience with elderly, physically disabled, developmentally disabled, or seriously mentally ill individuals
- Valid driver's license and ability to drive to conduct onsite visits
- Level One fingerprint clearance card for working with vulnerable adults
- Knowledge of HIPAA, federal and state statutes, and person-centered service planning
