Sat & Sun 8am-1pm every other weekend;
Weekend Day shift

Salary: $53.00 hourly

Provides, arranges for, teaches and/or supervises nursing care through the use of telehealth patient encounters, telephonic follow‑up calls, and patient visits. Routine weekend and holiday telehealth monitoring; PTO coverage for the Telehealth Supervisor as needed. Assists with TH program marketing.

• Provides/coordinates patient‑centered care delivery to patients receiving telehealth home monitoring.
• Promotes the successful adoption of patient disease self‑management behaviors.
• Program Promotion.
• Other duties as assigned.

• Minimum Education: After July 2012, requires a Bachelor’s Degree in Nursing or an Associate’s Degree/Diploma with proof of enrollment in a BSN program and completion within five years.
• Preferred Education: None.

• Required: Two years of Med‑Surg experience.
• Preferred: Two years of current home‑care experience; 1–2 years managing high‑risk patients (cardiac, pulmonary, diabetic).

• Required: Current CPR certification, NJ RN licensure, valid driver’s license.
• Preferred: Certification in specialty.

• Excellent physical assessment skills.
• Excellent communication skills.
• Familiar with Microsoft Office & Google Suite.
• Preferred: Knowledge of state and federal home‑health regulations.
