Pay Rate: $50.00 - $55.00 per hour

Schedule: Per Diem

We are looking for a licensed and compassionate Physical Therapist to join our team. In this role, you’ll provide in‑home therapy services that support patients' rehabilitation, mobility, and quality of life.

Role Overview

As a Physical Therapist, you will assess, plan, and deliver therapeutic interventions for patients recovering from injury, illness, or surgery. You’ll help patients regain mobility, manage pain, and safely perform daily activities all from the comfort of their homes.

Responsibilities
  • Conduct initial and ongoing assessments of patients’ physical condition and functional abilities
  • Develop personalized care and therapy plans in collaboration with the care team
  • Provide direct physical therapy services, including strength, balance, gait, and mobility training
  • Educate patients and caregivers on exercises, safety, and equipment use
  • Monitor patient progress, adjust care plans, and report outcomes
  • Recommend assistive devices and home safety modifications as needed
  • Collaborate with nurses, occupational therapists, aides, and social workers for coordinated care
The Ideal Candidate
  • Current Physical Therapist license
  • Graduate of an accredited Physical Therapy program (DPT or equivalent)
  • Minimum of 1 year experience in home health, rehab, or hospital setting preferred
  • Strong clinical, communication, and assessment skills
  • Able to work independently and manage a mobile caseload
  • Valid driver’s license and reliable transportation required
  • CPR certification and compliance with health requirements
