- Provide direct patient care with a harm reduction approach, focusing on HIV and Hepatitis C treatment
. - Conduct health assessments, diagnoses, and treatment planning
. - Prescribe medications and monitor patient progress.
- Educate and counsel patients on preventive care and chronic disease management
. - Collaborate with physicians and healthcare teams to ensure comprehensive patient care
. - Maintain compliance with CNO standards
, medical directives, and privacy regulations. - Document patient interactions in electronic medical records (EMR).
- Engage with community partners to enhance patient support services.
Registered Nurse Practitioner (NP-Adult or NP-Primary Health Care) with valid CNO registration.
Masters degree in Nursing (MN-NP) or Master of Science in Nursing (MScN).
Experience in primary care, harm reduction, HIV, Hepatitis C, or community health (preferred).
Strong clinical assessment, diagnostic, and prescribing skills
.
Excellent communication and patient education abilities.
Ability to work independently and collaboratively in a multidisciplinary team.
Proficiency in electronic medical records (EMR) systems
.
N95 Mask Fit Test Certification & up-to-date immunizations (MMRV, TB test, or chest X-ray).
