We are recruiting for a full‑time band 6 cpn to join our team on a secondment, working within the flintshire older persons community mental health team.

Please note that we will only accept applications from staff currently employed by betsi cadwaladr university health board.

as care coordinator, you will hold a caseload of patients with organic and functional mental health needs, travelling to see them in their own homes, in care homes and in‑patient settings.
