We are looking for a compassionate Physical Therapist to join our growing team! The right candidate has a can-do attitude, excellent interpersonal skills, and a current state PT license. In this role, you will be providing medical support to our patients while ensuring great customer service.

Responsibilities

Provide physical therapy services to patients according to a written physician's plan of care, including assessment and evaluation of therapeutic/rehabilitative/functional status and participation in the development of the total plan of care.

Assess muscle strength, mobility, gait, ROM, and potential for rehab.

Direct physical therapy treatment.

Instruct patients and families/caregivers in the use and care of therapeutic appliances.

Determine priority needs for physical therapy.

Report patient’s reaction to treatment or changes in condition to the physician.

Initiate physical therapy programs and instruct other personnel and/or family/caregivers, participating in case conferences.

Train patients in the use of prosthetic devices as necessary.

Identify needs for other home health services and refer as appropriate.

Prepare and submit clinical and progress summaries based on goal attainment.

Participate in discharge planning for patients.

Provide physical therapy consultation to home families/caregivers when indicated.

Provide in-service education programs for nursing personnel as needed.

Participate in peer consultation processes.

Supervise physical therapy assistants according to policy and regulations.

When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care biweekly.

Qualifications

Degree from an accredited physical therapy program.

Licensed to practice as a physical therapist in the state.

At least two years of relevant experience, including community/home health.

Good verbal and written communication and organizational skills.

Current CPR certification.
