• Plan and administer prescribed skilled physical therapy treatment and training for patients with varied injuries, illnesses, and functional disabilities to achieve the highest level of physical function.
• Test/screen patients’ physical strengths to assist the physician in evaluating patients’ level of function and records findings to develop or pursue treatment programs and establish measurable training objectives.
• Develop and implement a conditioning/rehabilitation program consistent with the physician’s Plan of Treatment and overall goals of the patient/rehab team. Adjust treatment as needed to achieve maximum results.
• Confer with physicians and the clinical team to obtain additional patient information and assist in developing, implementing, and revising therapy treatment programs and Plan of Treatment.
• Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aides that reflect current therapy plans. Supervise and evaluate staff performance and monitor documentation completion.
• Accurately, promptly, and thoroughly document patients’ care observations, interventions, and evaluations. Ensure interim orders from physicians and PTs are promptly documented and implemented.
• Report patient progress to physicians, Clinical Manager, staff, patients, and families. Submit evaluation, treatment plans, progress reports, and discharge summaries to supervisors and care management staff.

• Degree from an accredited Physical Therapy Program approved by the APTA.
• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Minimum of one year physical therapy experience preferred.
• Strong organizational and communication skills.
• Valid driver’s license, automobile insurance, and reliable transportation.

• Base pay: $85,400 – $117,500 per year.
• Pay per visit/unit: $54.00 – $76.00.
• Weekly hours: 40.
