Licensed Social Worker; LSW

We are seeking a compassionate and skilled Licensed Social Worker (LSW) to join our dedicated team at Briarwood Nursing and Rehab in Flint, Michigan. As a valued member of our community, you will play a crucial role in providing high-quality social work services to our residents.

As a Licensed Social Worker (LSW), you will work closely with our interdisciplinary team to assess, develop, and implement individualized care plans that promote the physical, emotional, and social well-being of our residents. You will:

• Conduct assessments and develop individualized care plans, ensuring that each resident receives personalized attention and support
• Provide counseling, education, and support services to residents and their families, addressing psychosocial and emotional challenges
• Develop and implement recreation and activity programs that foster social interaction, independence, and overall well-being
• Collaborate with other healthcare professionals to ensure seamless coordination of care and services
• Document resident progress, communicating effectively with family members and healthcare providers

• Valid Licensed Social Worker (LSW) certification in the state of Michigan
• Strong knowledge of social work principles, theories, and practices
• Excellent communication, interpersonal, and organizational skills
• Ability to work effectively in a fast-paced, dynamic environment
• Compassionate and empathetic approach to working with residents and their families
