• Assess patients' social, emotional, and psychological needs to develop personalized care plans
• Provide counseling, education, and support to patients and their families to promote optimal health outcomes
• Collaborate with interdisciplinary teams to ensure comprehensive care and effective communication
• Identify and address social determinants of health that impact patient outcomes
• Develop and implement community resources and interventions to support patients and families
• Maintain accurate and detailed records of patient interactions and care plans
• Participate in quality improvement initiatives to enhance patient care and outcomes

• A bachelor's degree in social work or related field (or equivalent experience)
