Center Well Home Health provides personalized, one-on-one home health care. You’ll work with a dedicated team to develop and manage care plans that support recovery and help patients regain independence at home.
Pay and ScheduleBase pay range: $70,500.00/yr - $96,900.00/yr
Pay range (per visit/unit): $45.00 - $63.00 — pay per visit/unit. Scheduled Weekly

The compensation range reflects a good faith estimate of starting base pay for full-time employment at the time of posting and may vary by geographic location and experience.
Responsibilities- Provide admission, case management, and follow-up skilled nursing visits for home health patients.
- Administer ongoing care and case management for each patient, with follow-up as directed by the Clinical Manager.
- Collaborate with the physician to develop the initial plan of treatment based on orders and assessments; implement, monitor, and revise the plan as required, under supervision.
- Coordinate care with physical therapists, occupational therapists, home health aides, and external providers.
- Report patient care/condition/progress to the physician and Clinical Manager on an ongoing basis.
- Implement the care plan with the patient and family to address needs and problems.
- Supervise Home Health Aides, Certified Home Health Aides, and Personal Care Workers; provide documented care plans and ensure completion of documentation.
- Discharge patients after physician and Clinical Manager consultation and complete required clinical documentation.
- Prepare medical documentation for all patients, including case conferences, patient contacts, medication changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports and share updates with other health care disciplines as needed.
- Diploma, Associate or Bachelor Degree in Nursing
- Minimum of one year nursing experience preferred
- Strong med-surg, ICU, ER, or acute care experience
- Home Health experience a plus
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Strong organizational and communication skills
- Valid driver’s license, auto insurance, and reliable transportation
