Assess plans and delivers nursing care in collaboration with professional Registered Nurses. This position will educate patients individually and in groups on disease processes and health prevention, and work collaboratively with others on large health initiative projects.

General Responsibilities
  • Performs general LPN nursing duties in providing care to patients. Gives direct patient care according to the therapeutic plan and individual patient needs as directed by the patient care plan.
  • Provides appropriate information to assist the Registered Nurse in the preparation and maintenance of an effective patient care plan.
  • Records in the patient’s official record a detailed report of care given which reflects specific aspects of the patient care plan.
  • Reinforces instructions given to the patient by the Physician and Registered Nurse and refers to new needs for instruction to the Registered Nurse.
  • Utilizes accepted protocols and procedures to aid in decision‑making.
  • Oversees appointment bookings and ensures preferences are given to patients in emergency situations.
  • Maintains timely flow of patients.
  • Prepares equipment and aids physician during treatment, examination, and testing of patients.
  • Arranges for patient testing and admissions.
  • Directs, responds, and refers to incoming telephone calls or inquiries.
  • Instructs patient and family regarding medications and treatment instructions.
  • Maintains and reviews patient records, charts, and other pertinent information.
  • Posts test and examination results in the patient’s cart.
  • Works collaboratively with provider, RN, and support staff to improve patient care and increase provider productivity by ensuring the smooth flow of patients.
  • Prepares and maintains records and reports.
  • Maintains exam rooms for necessary supplies and materials. Ensures cleanliness. Prepares a list of medical supplies as needed.
  • Works collaboratively with providers to identify critical concerns.
  • Works collaboratively with providers to identify patient education needs and organize patient education materials.
  • Performs audits of medical records and care for clinical quality.
  • Assists in the preparation for reviews by accrediting and funding authorities.
  • Performs other duties as assigned.
Physical Demands
  • While performing the duties of this job, the employee is regularly required to talk or hear.
  • The employee frequently is required to stand, walk, sit; use hands to handle or feel; reach with hands and arms; and stoop, kneel, or crouch.
  • The employee must occasionally lift and/or move up to 25 pounds.
  • Specific vision requirements include the ability to see at close range.
  • At times, may require more than 40 hours per week to perform the essential duties of the position.
  • Fine hand manipulation (keyboarding).
  • Travel between sites using your own vehicle.
Work Environment
  • Internal office space located within the clinical environment.
  • May be exposed to communicable diseases.
  • The noise level in the work environment is usually moderate.
Job Requirements

Education and/or Experience
  • Completion of an LPN program accepted for licensure by the Michigan Board of Nursing
  • Current State of Michigan LPN license
  • Minimum of two(2) years of professional nursing experience in a healthcare setting.
  • Current CPR certification
OR
  • Equivalent combination of education and experience requirements sufficient to successfully perform the duties of the job as listed above.

  • Must have a thorough knowledge of the theory and practice of healthcare management.
  • Must be able to communicate effectively orally and in writing.
  • Must be able to identify problems and recommend and institute solutions.
  • Excellent interpersonal and customer service skills, to effectively and tactfully deal with a diverse group of patients, staff, customers, and community groups.
  • Flexible, with the ability to work independently and with minimal guidance.
  • Strong oral and written communication and presentational skills.
  • Ensures that communication with staff, patients, and external contacts occurs with courtesy, tact, and a positive attitude.
  • Effectively communicates verbally with patients.
  • General computer proficiency.
  • Ability to communicate effectively with diverse populations.
