- Complete patient assessments and admissions
- Develop and oversee individualized care plans
- Conduct supervisory visits and ensure quality patient care
- Perform medication reconciliation and provide clinical oversight
- Mentor and support field clinicians
- Maintain compliance with state and federal regulations
- Step in to provide hands-on patient care when needed
- Active Registered Nurse (RN) license in the State of Michigan
- Experience in private duty nursing
- Experience overseeing field-based clinical staff
- Current CPR certification
- Ability to provide hands-on patient care as needed
- Willingness to travel within the service area
- Reliable transportation and a valid driver’s license
- Eligible to work in the United States
- Leadership or supervisory experience in home health or private duty nursing
- Experience with quality improvement or clinical program support
- Minimum 2 years of home health care experience
- Knowledge of state home care regulations, supervisory visit documentation requirements, and private duty care standards
- Experience with charting and care plan documentation
