Licensed Practical Nurse, Home Health

Overview

Join Center Well Home Health as a Licensed Practical Nurse (LPN). You will provide personalized, one-on-one care in patients' homes, developing and managing care plans in collaboration with physicians and clinicians to support recovery and independence.

$44,600.00/yr - $61,400.00/yr

• Provide professional nursing services under the direction of an RN in compliance with the state Nurse Practice Act, licensure/certification requirements, healthcare standards, laws and Center Well policies.
• Deliver skilled nursing interventions for illness, rehabilitative needs and preventative care, including teaching and training in the plan of care.
• Apply knowledge and skills to support problem resolution and achieve patient goals and outcomes.
• Consult with the RN Supervisor regarding changes to the Plan of Treatment and communicate physician orders as permitted by law/regulations/Nurse Practice Act.
• Implement the nursing plan of care using an individualized, goal/outcome-driven approach.
• Maintain timely communication with patients, physicians, and the healthcare team regarding needs and status changes; participate in care coordination and discharge planning as appropriate.
• Maintain confidentiality and professional conduct in handling patient information.
• Ensure availability of supplies and equipment to safely implement the plan of care, and document visit notes accordingly.

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.

• $44,600 - $61,400 per year base pay
