Center Well Home Health – Baylor Registered Nurse, Home Health

Center Well Home Health provides a base pay range of $70,500.00/yr to $96,900.00/yr for experienced registered nurses applying to the Baylor Home Health Nurse role.

• Provide admission, case management and follow‑up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient and provide necessary follow‑up as directed by the Clinical Manager.
• Confer with physician in developing the initial plan of treatment based on physician orders and initial patient assessment; revise plan in consultation with physician as needed.
• Coordinate appropriate care with physical therapists, occupational therapists, home health aides and external providers.
• Report patient care/condition/progress to physician and Clinical Manager on a continuous basis.
• Implement patient care plan in conjunction with patient and family to assist in achieving optimal resolution of needs/problems.
• Coordinate/oversee/supervise the work of home health aides, certified home health aides and personal care workers and monitor the appropriate completion of documentation.
• Discharge patients after consultation with physician and Clinical Manager, preparing and completing needed clinical documentation.
• Prepare appropriate medical documentation on all patients, including case conferences, medication order changes, re‑certifications, progress updates and care plan changes; prepare visit/shift reports and confers with other health care disciplines.

• Diploma, Associate or Bachelor Degree in Nursing.
• Minimum of one year nursing experience preferred; strong medical/surgical, ICU, ER, acute experience.
• Home Health experience a plus.
• Current and unrestricted Registered Nurse licensure and current CPR certification.
• Strong organizational and communication skills; valid driver’s license, auto insurance and reliable transportation.

• $45.00 - $63.00 – pay per visit/unit.
• $70,500 – $96,900 per year base pay.
